Forms & Charges
To pre-register, the individual must complete BOTH the Statistical Information Sheet and have the Legal Next of Kin (spouse if married or majority of living adult biological children) sign the Authorization for Cremation. Those documents must be returned to our office, either in person or by U.S. Mail, with the one time, non-refundable $35.00 fee PRIOR to the death of the individual. For clients that are not pre-registered with us, the charge is $ 1,275.00, and for those who pre-register with us, our charge is $ 950.00. Our charges include: removal of the deceased from the place of death, the cremation container the individual is placed in for cremation, the cremation process itself, the return of the cremated remains in a solid container suitable for storage (or burial depending on the requirements of individual cemeteries), completing and filing a death certificate, obtaining the required physician's signature on the death certificate, obtaining the signature of the Medical Examiner on the Cremation Permit, notification of Social Security, assistance with DHHS applications, scheduling of burial or entombment at a National Cemetery, and direction of family to appropriate County Veteran's Affairs Officer for assistance with paperwork for County Burial Benefits for Honorably Discharged Veterans.
We provide service to clients in all Mid and Northern Michigan counties, however, there is no mileage charge for those within our service area which includes: Arenac, Bay, Midland, Saginaw, Tuscola, Gladwin, Genesee & Shiawassee Counties. For other counties a minimal mileage charge will apply. Additional assistance with urns, urn vaults, obituaries, memorial folders, holy cards, etc. is also available. Please note that we do not offer caskets, hearse, embalming, public viewing.
